Die Nutzung des IPOS ist kostenfrei, es wird aber eine (kostenfreie) Registrierung auf der POS-Website (www.pos-pal.org) gewünscht. Um die Fragen möglichst kurz zu halten, wird nur „der Patient“ genannt: Gemeint ist immer „ die Patientin/der Patient". Zur Bewertung der Integrated Palliative care Outcome Scale (IPOS): Die IPOS-Ergebnisse können genutzt werden für die Berechnung i)eines individuellen Item-Punktwertes ii)von Subskalen iii)eines IPOS-Gesamtpunktwertes. Diese werden wie folgt beschrieben: i)Individuelle Item-W erte sind hilfreich, um einzelne Dimensionen, z. B. Schmerz, Sorgen/Angst oder Informationsbedarf im zeitlichen Verlauf darzustellen.Der IPOS besteht aus zehn Items, die folgende Dimensionen bei fortgeschrittener Er-krankung erheben: die körperliche, emotionale, psychologische und spirituelle Dimension, sowie die Bereitstellung von Informationen und praktischer Hilfe. Für die Mehr-zahl der Items sind fünf Antwortmöglichkeiten vorgegeben, von denen jeweils nur eine pro Frage ausgewählt werden soll. Frage 1 ermöglicht es dem Ausfüllenden, die Hauptprobleme zu benennen. Diese offene Frage wird nicht mit Punktwerten bewertet, bietet aber einen Einblick, welche Hauptprobleme oder -sorgen bestehen und wie diese sich im Verlauf verändern. Frage 2 fragt nach zehn spezifischen Symptomen. Für jedes gibt es fünf Antwortmög-lichkeiten (bewertet mit Punktwerten zwischen 0 und 4). Dabei ist zu beachten, dass sich die Frage darauf bezieht, wiesehr der Patient von dem Symptom BEEIN-TRÄCHTIGT war, nicht darauf, wie stark ausgeprägt das Symptom war. Ebenfalls zu beachten ist, dass der IPOS für Mitarbeiter zusätzlich über die Antwortmöglichkeit „nicht beurteilbar“ verfügt. Wenn diese Antwortmöglichkeit angekreuzt wird, muss dies entsprechend vermerkt werden und darf nicht in die Berechnung eines Punktwertes eingehen. Ebenso dürfen auch nicht beantwortete Fragen nicht in die Berechnung eines Punktwertes eingehen und müssen entsprechend als „fehlende Werte“ vermerkt werden.Sollten über die in Frage 2 gelisteten Symptome hinaus am Ende der Frage 2 zusätz-liche Symptome genannt werden, können diese mit derselben Skala wie zuvor (Punktwert zwischen 0 und 4) bewertet werden. Soll der Punktwert eines zusätzlichen Symptoms genutzt werden, um den Verlauf des Symptoms abzubilden, muss beachtet werden, dass es sich zu unterschiedlichen Erhebungszeitpunkten um dasselbe zusätzliche Symptom handelt und in der gleichen Reihenfolge dokumentiert wird (bei mehreren zusätzlichen Symptomen). Frage 3 bis 9 beziehen sich auf psychologische, spirituelle und praktische Symptome und Probleme sowie solche, die die Kommunikation betreffen. Für jede Frage gibt es fünf Antwortmöglichkeiten (bewertet mit Punktwerten zwischen 0 und 4). Zu beachten ist, dass der Text der Antwortmöglichkeiten zwischen den Fragen variiert. Die Fragen 6, 7 und 8 beziehen sich eher auf positive Erfahrungen (z.B.: „Haben Sie so viele Informationen erhalten, wie Sie wollten?“) anstatt auf Sorgen (z.B.: „Waren Sie traurig bedrückt?“). Entsprach der inhaltliche Wert bei den vorherigen Antworten dem numerischen (0 = gar nicht, 4 = immer), so sind die Antwortmöglichkeiten hier in GEGENLÄUFIGER REIHENFOLGE (0 = immer, 4 = gar nicht). Der höchste inhaltliche W ertentspricht damit dem geringsten numerischen. Manche Anwender fragen, ob dies geändert werden kann. Wir haben die Erfahrung gemacht, dass der Ausfüllende die Fragen überlegter beantwortet. Es ist allerdings hilfreich, wenn dies durch einen Absatz zwischen Frage 5 und 6, sowie zwischen Frage 8 und 9 hervorgehoben wird (siehe IPOS Antwortbogen unter Downloads auf www.pos-pal.org). So soll sichergestellt werden, dass die einzelnen Antwortoptionen nochmals gelesen werden, statt fälschlicherweise davon auszugehen, dass diese gleich geblieben sind. Frage 10 geht nicht in die Bewertung bzw. den Punktwert ein. Sie gibt an, wer den Fragebogen ausgefüllt hat, damit dies bei der Interpretation berücksichtigt werden kann. ii)Psychometrische Daten zu IPOS-Subskalen sind noch nicht publiziert. Die Ergebnisse der bereits abgeschlossenen IPOS-Validierungsstudie zeigen jedoch drei Faktoren bzw. „Konstrukte“ auf, die Subskalen innerhalb des IPOS untermauern: a. Die Subskala Körperliche Symptome–diese besteht aus acht IPOS-Items (Schmerz, Atemnot, Schwäche oder fehlende Energie, Verstopfung, Mundtrockenheit oder schmerzhafter Mund, Schläfrigkeit, eingeschränkte Mobilität und Sorgen der Familie und Freunde). Der Punktwert dieser Subskala kann daher zwischen 0 und 32 liegen. b.Die Subskala Gastrointestinale Symptome–diese besteht aus drei IPOS-Items (Übelkeit, Erbrechen und Appetitlosigkeit). Der Punktwert dieser Subskala kann daher zwischen 0 und 12 liegen. c.Die Subskala Psychologische und praktische Probleme oder Beschwerden sowie solche, die die Kommunikation betreffen–diese besteht aus sechs I-POS-Items (Sorgen/Angst, depressive Stimmung, Gefühl, im Frieden mit sich selbst zu sein, die Möglichkeit, Gefühle teilen zu können, Informationsbedarf und praktische Angelegenheiten). Der Punktwert dieser Subskala kann daher zwischen 0 und 24 liegen. Zu beachten: Wenn ein Item, der Komponente einer Subskala ist, mit „nicht be-urteilbar“ beantwortet wurde oder fehlt, kann kein Subskalen-Punktwert errechnet werden.Daten zu den IPOS-Subskalen werden bald publiziert. Er entspricht der Summe der Punktwerte aller 17 Fragen. Er kann daher zwischen 0 und 68 liegen. Zu beachten: Frage 1, die zusätzlich angegebenen Symptome am Ende der Frage 2 und Frage 10 gehen nicht in die Berechnung des Gesamt-Punktwertes ein. Generell zu beachten: Der Gesamt-Punktwert kann wichtige Probleme verbergen und sollte daher nur ZUSÄTZLICH zu individuellen Symptom-Punktwerten angegeben werden. Ein niedriger IPOS-Gesamt-Punktwert kann beispielsweise einen oder mehrere Einzel-Items beinhalten, die als stark (Punktwert 3)oder extrem stark (Punktwert 4) eingestuft wurden. Wird ein Item als „nicht beurteilbar“ bewertet oder fehlt ein Item, so kann kein Gesamtscore ermittelt werden.
