The pneumonia severity index (PSI) is a clinical prediction rule to calculate the probability of morbidity and mortality among patients with community acquired pneumonia. Patients in risk group I - III (<70 - 90 points) can be considered to get an outpatient therapy. Patients in risk group IV or V (91- >130 points) should be treated in hospital. N Engl J Med. 1997; 336:243-250 Der PSI erlaubt eine sichere Klassifizierung in Patienten mit einem niedrigen Letalitätsrisiko (Risikoklassen I – III, maximal 3% Letalität), mit erhöhtem Risiko (Risikoklasse IV, ca. 8% Letalität) sowie Patienten mit einem hohen Risiko (Risikoklasse V, ca. 35% Letalität) und ermöglicht eine risikoadaptierte Auswahl des Behandlungsortes sowie die Identifizierung derjenigen Patienten, die mit einem minimalen Risiko ambulant behandelt werden können.

Ab CRB-65 / CURB = 1 sollte unter Einbeziehung anderer Risikofaktoren die stationäre Aufnahme erwogen werden.
