Lifelines collects questionnaire data, measurements, and biological samples from the entire cohort in rounds or waves called general assessments. The population cohort includes more than 167.000 participants, including children, adults, and elderly, over a 30-year period. Children follow a special child protocol until they turn 18. Elderly participants follow the adult protocol plus some additional questions (in assessments 1A, 1B and 1C) and additional measurements (MMSE). Assessment 1A or “Baseline Assessment” is the first general assessment of Lifelines. Assessment 1A was performed between 2007 and 2013, with the majority of participants being assessed between 2010 and 2013. Assessment 1A was followed by follow-up questionnaires 1B and 1C, and by second assessment 2A. For adult and elderly participants, assessment 1A consisted of the following elements: - 1A Visit 1 (physical measurements, MINI, MMSE) - 1A Visit 2 (blood and urine samples) - children followed an age-dependent program Assessment 2A or “Second Assessment” is the second general assessment of Lifelines. Assessment 2A was performed between 2014 and 2017. Assessment 2A was preceded by general assessment 1A, and is followed by follow-up questionnaire 2B and general assessment 3A. For all participants, assessment 2A consisted of the following elements: - 2A Visit 1 (physical measurements) - 2A Visit 2 (blood and urine samples) - children followed an age-dependent program Adult and elderly Lifelines participants participating in the second general assessment 2A received one follow-up questionnaire, 2B. Questionnaire 2B was sent out from 2015 until the end of 2019. For adult and elderly participants, this assessment also consisted of the following elements: - 2A Questionnaire 1 - 2A Questionnaire 2 Children received an age-dependent set of questionnaires during assessment 2A. Assessment 3A or “Third Assessment” is the third general assessment of Lifelines. Assessment 3A will be performed between 2019 and 2023. Assessment 3A was preceded by general assessment 1A and general assessment 2A. For all participants, assessment 3A consists of the following elements: - 3A Visit 1 - 3A Visit 2 For adult and elderly participants, this assessment also consists of the following elements: - 3A MINI - 3A Questionnaire 1 - 3A Questionnaire 2 - 3A Questionnaire 3 This document contains all sections of the 1A Questionnaire 1 for adult patients (18-65 years of age). In this Lifelines Questionnaire Questions 3, 5, 6, 7, 9, 11, 13, 14, 15, 17,18, 19, 21, 22, 23 were used. They are aggregated in this item. Please fill in the answer for each question separately (e.g. Question 3: 0; Question 5: ...) The Self-Assessment Questionnaire of INTERMED (IM-SA) 1) is an instrument to identify individual (complex) health care needs. This instrument assesses biopsychosocial complexity, health care needs and barriers to receive standard care. Lifelines participants of 65 years and older received (parts of the) Intermed in assessment 1A and 2A(section: elderly). Background The IM-SA score indicates an individual level of complexity of health care needs. Complexity is subdivided in four domains: Biological, Psychological, Social and Health System. Every domain is divided into three time segments: History, Current State and Vulnerability/Prognosis. All IM-SA items are scored on a four-level rating scale. The rating scores range from 0 to 3: 0 Zero evidence for a symptom, disturbance or health care need 1 Mild vulnerability/need for monitoring or prevention 2 Moderate vulnerability/need for treatment or inclusion in treatment plan 3 Severe vulnerability/need for immediate or intensive treatment For an overview of possible answers and associated scores, see here: IM-SA scores The total maximum score of the IM-SA is 60. The complexity cutoff point of the IM-SA is 19, i.e. a score of 19 or higher indicates complexity.
