aseptisch = Nichtinfiziertes OP-Gebiet, keine Entzündung vorhanden, ohne Eröffnung von Respirations-, Gastrointestinal- oder Urogenitaltrakt. Primär verschlossen, ggf. mit einer geschlossenen Drainage versorgt. Operative Wunden nach stumpfen, nicht penetrierenden Traumata, sofern die o. g. Kriterien erfüllt sind. z. B. elektive Schilddrüsen-, Herz-, Gelenk-OP. bedingt aseptisch = Eröffnung von Respirations-, Gastrointestinal- oder Urogenitaltrakt unter kontrollierten Bedingungen und ohne ungewöhnliche Kontamination. z. B. Appendektomie oder OP im Bereich des Oropharynx, der Vagina oder der Gallenwege, sofern keine Hinweise für lnfektionen oder Verletzungen der aseptischen Technik vorliegen. kontaminiert = Offene, frische Zufallswunden; Operationen mit einem größeren Bruch in der aseptischen Technik (z. B. offene Herzmassage) oder mit deutlichem Austritt von Darminhalt; Eingriffe, bei denen eine akute nichteitrige Entzündung vorhanden ist. z. B. abdominoperineale Rektumamputation. septisch = Alte Verletzungswunden mit devitalisiertem Gewebe; Eingriffe bei bereits vorhandener lnfektion oder nach Perforation im Gastrointestinaltrakt. Operationsfeld ist schon präoperativ mit Erregern von möglichen postoperativen lnfektionen besiedelt. z. B. OP nach Darmperforation oder bei eitriger Cholezystitis, Klappenersatz bei florider Endokerditis.
