Dieser Bogen liegt jedem Krankenblatt bei. Ist während des Krankenhausaufenthaltes keine NI aufgetreten, nur Personalienfeld ausfüllen und nein ankreuzen. Bei Auftreten einer NI ist der gesamte Fragebogen auszufüllen. Den Bogen anschließend zurück in die Patientenaufnahme /-abmeldung geben.
